Get a catalog. When you enroll in or inquire about Extension courses your name is
placed on our
mailing list and you will automatically receive a catalog each term for the next two years. If you
receive more than one catalog, please help us reduce waste by clipping the mailing label from the
extra copy and sending it to UC Berkeley Extension, CMS Dept., 1995 University Ave., Suite 110,
Berkeley, CA 94704-7000, so that we can remove that entry from our list.
Change of address. To change your address, send us the mailing label along
with a completed
Change of Permanent Student
Information form.
Remove from list. UC Berkeley Extension never sells its mailing list, but if
you would like to be
removed from the list, please send us the mailing label with your request. This is the most
effective way to ensure we are able to comply with your request.
