Records

We maintain permanent academic records for UC Berkeley Extension students who have completed credit courses, and noncredit courses numbered 800-899, and for students who ask us to record their noncredit continuing education unit (ceu) study.

Requesting a Transcript

Please allow 10 business days to process and mail your transcript(s), from the time we receive your request. (During our peak period, November-March, fulfillment may take up to 15 business days.) If you need your transcript(s) mailed sooner, you must specify rush service. Rush service ensures that your transcript(s) will be mailed within one business day after the receipt of your request. All transcripts are sent via First Class U.S. Mail.

Transcript Request form. You can mail or fax your request with the appropriate payment to the address or fax number listed at the bottom of the form. No phone or e-mail orders can be accepted.

2. Provide the information outlined in the steps below. Failure to include all required information will delay processing of your transcript request.

No partial transcripts will be issued. UC Berkeley Extension no longer issues "In-Progress" transcripts. Students may use either the Enrollment Confirmation or request a "Statement of Fees Paid" letter to confirm their current enrollment status.

Hours and Fees: $15 per copy requested for regular delivery. There is a $15 additional fee for Rush Service, which may be either in-person pickup at the Registrar's Office at 1995 University Ave., Suite 110, Berkeley, or mailing via First Class U.S. mail, provided the request is made before 3:45 pm, Mon.-Fri., with the exception of holidays. Rush requests submitted after 3:45 pm will be processed and mailed the next business day. Please include your daytime and evening phone numbers in the event any questions arise with respect to your request.

In-Person Order and Pick-up of Your Transcript

Walk-in purchase of transcripts is permitted, with no advance order, at 1995 University Ave., Suite 110, Berkeley, 8 am-3:45 pm, Mon.-Fri. You must show valid photo ID when picking up your transcript. If someone else will pick up your transcript, you must provide that person with your written and signed authorization in order for your transcript to be released; your authorization will be retained by Extension. Your designee must present valid photo ID. There is an additional fee of $15 for this rush service.


Course Description/Enrollment Verification

1. Course descriptions are photocopied from the applicable UC Berkeley Extension catalog. There is an $8 charge for each copy of each course description requested.

2. There is a $15 charge for written verification of a student enrollment or student record provided to a third party. Release of information to a third party requires written authorization from the student. Please submit any necessary forms that must be completed.


A Note About Privacy

The UC Berkeley Extension Dean's Office is responsible for maintaining Extension student records, and any questions about them should be addressed to the Dean's Office in writing. The policy governing the disclosure of information from student records is available at
http://campuspol.chance.berkeley.edu/policies/studentrecdisclosure.pdf. The policy provides for the right of students to be informed about where student records are located, to review their own records, to request corrections, to grieve alleged violations of privacy, to consent to disclosures of personally identifiable information, and to file complaints with the U.S. Department of Education. The Dean's Office has designated student names, terms of attendance, major fields of study, and certificates earned as directory information under the policy; please note that Extension's directory information is more restricted than that for matriculated students.

Your Social Security Number

The University is required by federal law to report your Social Security number and other pertinent information to the Internal Revenue Service pursuant to the reporting requirements imposed by the Taxpayer Relief Act of 1997. Social Security number disclosure is mandatory.
