Continuing Care and Case Coordination in Chemical Dependency
X134 (2 semester units in Public Health)
This course provides an overview of continuing care, case coordination principles, and the practices used to establish and maintain recovery and prevent relapse. The purpose of the course is to expose participants to different ways of thinking about the recovery and potential relapse process for chemically dependent clients, and to identify effective treatment and case coordination approaches for these clients. Topics aim to serve a diverse population.

ALAN LAZERE, M.S.W., LCSW, is the manager for mental health and chemical dependency services for Kaiser Permanente in the Diablo service area. He is the cofounder and former program director of the Chemical Dependency Recovery Program at Kaiser Permanente in San Francisco.
